As Otto said to Marisol, the "Swedish éclair" pastry eaten in the cafe' is called semla. This is a nod to the Swedish origin of the novel on which the movie is based. The pastry, however, is not mentioned in the book. It is also not commonly found in Pittsburgh, roughly where the American movie takes place.

Otto's character traits are highly emblematic of autism since autistic people tend to have very literal thought patterns toward everything, which is exemplified by Otto's argument with the staff at the hardware store over his rope purchase.

Sonya was pregnant at the time but lost her baby and is shown in hospital, weeping at their loss. She suffered major injuries and became a paraplegic. Otto had made a cot for the baby which he'd kept and now gives to Marisol for her new baby. At the end of the movie Otto dies from a heart condition.

Despite being the son of two legendary film stars in the form of Hanks and his mother Rita Wilson, the younger Hanks never displayed an interest in acting up until his role in A Man Called Otto.

About 10% of the cat ended up being all in-camera and about 20% CG cat. The rest are cat comps, greenscreen plates comped in to the photography. JAMM Visual, led by Kevin Pierce, completed all of the cat work. They did the CG cat and they did all of the cat comps, which turned out very well.

The film opens six months after Otto's cherished wife Sonya (Rachel Keller) has died of cancer. In flashbacks, we learn how Sonya and Otto met, all they loved about each other, and the horrific crisis they faced together.

Lorianne Burgess was in shock when she found out Tom Hanks was going to film "A Man Called Otto" inside of her bakery in Ambridge, Beaver County. She had to snap out of it fast because Hollywood also asked her to come up with a special recipe for a Swedish pastry called a Semlor.

At one point, she brings him food in Tupperware dishes like chicken mole and Salvadorian cookies called salpores. Otto eats the mole, which he finds tasty, before attempting suicide.
